Wellness Ability Customer Feedback Form

Your feedback is important to us.
If you believe a serious breach has occurred, please refer to your Client Handbook for guidance on how to report the matter directly to the appropriate authorities.

This form is intended for NDIS Participants, their families, carers, and friends who are receiving services through Wellness Ability. Its purpose is to help us maintain a service that is fair, inclusive, and transparent.

We encourage you to provide as much detail as possible, as this enables us to keep our services relevant and to address any concerns effectively.

If you suspect a serious breach of practice, please consult your Client Handbook and report the incident directly to the relevant external agencies.
